Office Policies





LATE CANCEL/ FAILURE TO KEEP APPOINTMENTS
We see all patients on an appointment basis, and ask that you call in advance so that we may reserve the time for you. The office phone number is (718) 520-8585. We try to see all patients on time, and request that you extend the same courtesy to us.

If you are unable to keep a scheduled appointment, call (718) 520-8585 and please notify us immediately. We request 24 hours notice so that this time may be given to another patient.

Failure to show for a scheduled appointment or failure to give a 24 hours notice shall result in a $30 fee. Please note that this fee is not covered by your insurance, and will be assessed to all patients, regardless of insurance carrier. This will also need to be paid before any Physical appointments will be made (this does not include sick visits). Regretfully, we have been forced to institute this fee due to the large number of “no shows” and late cancellations we have encountered.


EMERGENCY CARE
In the event of a severe emergency, go immediately to the emergency room at the nearest hospital. Call our office to notify the receptionist of Emergency Room visit.


TELEPHONE CALLS
The doctors and staff are available for questions you may have concerning your health care problems; however, it would be unfair to other patients if the doctor were to disrupt treatment to answer every phone call. Our staff is trained to answer most questions. If your telephone call requires that the doctor speak with you, please leave a message and the doctor will return your call at the earliest opportunity.


PRESCRIPTIONS AND RENEWALS
All prescriptions and authorizations for renewals should be requested during normal office hours. All prescriptions will be called in to the pharmacy of your choice by the end of the business day.


FEES AND PAYMENTS
We make every effort to keep down the cost of your medical care. Unless you have made prior advance arrangements, payment is expected at the time of service regardless to who is bringing your child in for the visit. In the event that payment is not made at the time of service, a $10 service charge will be added to your account balance. If your check is returned to us for insufficient funds, we will assess a $30 service charge to your account to defray fees charged to us by the bank. All accounts sent tio collection agency will be charged a 35% service fee for collecting overdue accounts.


MINORS
A parent or legal guardian must accompany a patient under age of 18 years on every visit to our office.


WELL VISITS
Your child will be seen by your pediatrician within 2-3 days after hospital discharge, at 2 weeks, 1,2 4,6,9,12, 15, 18 month, 2 years, 2 1/2 years. At 3 years and after annually through age 21. SICK VISITS
If your child becomes sick or injured or you have any concern please call office as soon as possible to schedule an appointment. We will try to see your child the same day.


AFTER-HOURS AND EMERGENCIES:
In case of emergency call 911 or take your child to the nearest Emergency Room. We have a pediatrician on call 24/7. Please call 718-520-8585 and follow instructions.


CHECKING IN FOR YOUR VISIT
We suggest that you try to come a few minutes early for your appointment. Office staff will will check you in. One of our office staff will verify your address, phone number and insurance information for accuracy.Please bring your child's insurance card to each appointment. We will also collect your co-pay and any balance due before your child is seen. If you would like us to bill you for the co-pay, we will charge a fee to help cover the cost of the billing.

Please call 718-520-8585 to make an appointment. Please fill out registration forms, sign and bring them with you for the first appointment.. At that first appointment we ask yiou to arrive early so we can complete registration. Please do not forget to make subsequent appointment before leaving the office.

For older child please do not forget to bring immunization records with you and forward a copy of your child's previous medical records.

Insurance card must be brought for each appointment.

We do charge a fee for appointments not cancelled 24 business hours prior to appointment.


REFERRALS
Many insurance companies require you to receive referral from you pediatrician (PCP) before you take your child to a specialist.

Please be aware that insurance companies can deny payment for services even if you have a referral.Please call your insurance company directly to find out about covered services.

When you call a specialist to make an appointment be sure to ask for the provider ID number for your insurance plan and confirm that the specialist is a member of your insurance network, otherwise you my be subject to out of network charges.
